Initial Consultation and Home Visit

The first service that the care manager does is the initial consultation and Home Visit.

We come to your home or other place where you will be the most comfortable to have your first meeting with the care manager.

During this meeting we will:

Discuss various aspects of care management

E

Determine which of these are appropriate for your situation and needs

E

Refer others to help you with your needs, if needed.

During this time, we establish our relationship so that care management services can be provided quickly, correctly and efficiently.

The assessment includes a family meeting and home visit to determine needs and gather the information we need to be ready to meet any urgent needs.

We discuss all the different parts of care management and help you decide which are the most appropriate for your situation and needs. Some of these will apply to your situation and others will not.

But it’s always best to know about all the available resources. In this way if there any services that might be helpful, care manager goes over all of these details with you.

Our care manager thoroughly explains all of this to you. We take the time to answer your questions. We even go over topics of questions that you should ask us. We go over the answers to those questions with you as well!

We really want you to fully understand everything about what we do and how we can help you, and determine exactly what you need from us. If we can’t provide everything that you need, we will refer others to provide those needs to you.

We have a large network of other professionals that we refer our clients to so that they are cared for and satisfied for all of their care management services.

And once we explained everything to you in this initial consultation answered all of your questions we can proceed to the next step of our care manager services – which is a complete assessment.

Now please be advised that you can always ask us about any of these steps in the care management process at any time. We want you to feel comfortable with us and make our relationship with you one that you and your family will cherish!
