In essence, here the care manager will screen, arrange, and monitor home assistance for you and other associated services that you may need.

This Is the Most Important Stage – Your Care Management

This is a stage where you will notice that you’re being cared for on a regular basis by our team of skilled individuals. Your care management can include accompanying you on visits to doctors or other aging specialist.

Your care manager will monitor the effectiveness of the plan and services including reassessments when indicated – to make sure what we are doing for you is getting you the desired results.

We do our best to ensure that our clients do not receive less care or services than they need or pay for more than is required or necessary. Our monitoring services provide regular visits to our client’s home or nursing facility, crisis management on a 24 hour basis, supervising and monitoring in-home caregivers, responding to our client’s changing needs, monitoring the quality and cost effectiveness of care.

Our Care Manager also provides regular updates for family and suggestions for change.

This includes friendly visitations to minimize isolation and a “familiar” face on a regular basis.
