Our Care Manager can help your family during a crisis by arranging a plan of action to resolve a specific crisis or problem. Our plan of action assists families in determining the services and care needed for our clients by considering the overall goals of:

Our Care Management services are 24 hours a day, 7 days a week.

You can rest assured that we have become very good at balancing the tasks that are needed to defuse a crisis that may come up in our client’s life.

We strive to make resources available to our clients and your family that allows our clients to remain safely in the community. We work with you and your family to offer the least restrictive intervention possible with the purpose of having the least amount of disruption to our client’s life.

Crisis intervention with geriatric care management can be a very delicate and emotionally charged event. Since we have managed many crisis interventions of various types; and with our many resources at our disposal, we are able to give you and your family the very best of outcomes if these situations arise.

Help is available anytime you need it, whether it is a household emergency to solve a problem at home or a trip to the emergency room for a threatening health condition that may be causing our client some other emergency in their lives.

Naturally during such times, we communicate with you and your family and all the other appropriate parties to minimize and handle the crisis in the best way possible for you.
