The care plan includes:

a Workable Plan of Action to get the desired results.

This plan will ensure that all of the following parties are in agreement and will be working in harmony for the outcome that is best for you. These parties are:

Families

Aging Individuals, and

E

Health Care Providers

We want to make sure that all on the same page regarding health care and living arrangements.

And we want to make sure that each knows his or her role in managing the needs of the aging individual.

The main goal of the care plan is to evaluate the current and future:

Emotional – needs of the aging individual.

Once this is done, we develop a plan to provide the appropriate assistance.

Access to support services

In all of this care plan development, our goal for you and your family is to maximize autonomy, independence, dignity and safety for our client and your loved ones.
